The value of accurate clinical documentation has gained a strong foothold in recent years, mostly due to its impact on high visibility metrics in a performance-based health care environment. As physicians juggle increasing administrative and clinical tasks, clinical documentation accuracy, a necessary evil, is often a source of frustration. This frustration stems partly from their interactions with documentation queries, the predominant approach to capturing severity for typical clinical documentation improvement (CDI) programs that typical CDI programs leverage.
