HAP's Achievement Awards

The Achievement Awards program recognizes health care providers for innovative patient-centered initiatives that improve health care quality, efficiency, and the health of their communities. Participation is a great way to exchange ideas and celebrate innovation!

Submission Portal

Follow the link below to log into your HAPonline.org account and submit your entry. The submission portal will be open from December 5, 2024, until the deadline January 21, 2025 at midnight.

To see how your organization's submission should be structured, please reference an example of a previous winning submission. Past Achievement Award winners also are highlighted in HAP's Resource Center; search keywords "Achievement Awards."

2024 Award Winners

The awards are selected in five categories:  Community Champions, Optimal Operations, In Safe Hands, Excellence in Care, and Workforce Innovators across two divisions (small/medium and large organizations).The prestigious Living the Vision award is chosen from all project submissions.
 

The 2024 Achievement Awards ceremony was held on April 3, 2024, during HAP's annual Leadership Summit. See the list and read the winning submissions.
