HAP's Achievement Awards

The Achievement Awards program recognizes health care providers for innovative patient-centered initiatives that improve health care quality, efficiency, and the health of their communities. Participation is a great way to exchange ideas and celebrate innovation! The submission deadline for the 2025 Achievement Awards program has passed. The next submission period will open during December, 2025.

The awards are selected in five categories across two divisions (small/medium and large organizations):

• Community Champions
• Optimal Operations
• Workforce Innovators
• In Safe Hands
• Excellence in Care

The prestigious Living the Vision Award will be chosen from project submissions. This award is focused on innovation and bold interventions that transform the patient experience, address overall health, and improve value. Entries for this award should ultimately answer the question: “What are we doing to live the vision of a Healthy Pennsylvania for All?”

2024 Award Winners

The awards are selected in five categories:  Community Champions, Optimal Operations, In Safe Hands, Excellence in Care, and Workforce Innovators across two divisions (small/medium and large organizations).The prestigious Living the Vision award is chosen from all project submissions.
