Innovative and Collaborative Post-Acute Care Strategy of Two Large Hospital Health Systems Through Their Joint Venture Medicare Shared Savings Program Accountable Care Organization
In an effort to better coordinate care, improve outcomes and lower costs, Jefferson Health and Main Line Health developed a new accountable care organization for post-acute care.
With its new network of preferred providers, the health systems focused on data-driven, comprehensive strategies to manage care for patients in skilled nursing facilities, home health, hospice, and outpatient rehabilitation therapy. The project was a considerable undertaking, as the health systems care for about 250,000 patients.
The post-acute care initiative had several key components:
- Development of a Post-Acute Care Steering Committee
- Using detailed Medicare claims analysis and publicly reported outcomes data to measure spending and outcomes by network providers
- Development of preferred partnership networks with skilled nursing facilities, home health, hospice, and outpatient rehabilitation therapy services
- Web-based, real-time automation and evidence-based clinical guideline tools to better coordinate care
- Continuously enhanced objective accountability measures for performance and collaboration
Through the project, the hospitals noted increased patient satisfaction while achieving a $13 million reduction in combined skilled nursing facility, home health, and hospice spend during 2018. Elements of the collaborative project have been recognized at the national level and have resulted in published manuscripts, as well.
