Closing Behavioral & Physical Health Care Gaps for the Betterment of Whole Person Care

Location: Online

Registration Deadline: January 31, 2023

Overview

Thanks to the underwriting support of HAPevolve and Bamboo Health, a HAPevolve endorsed partner, this webinar is complimentary for all HAP members.

Overview

People are not just the sum of their clinical history or behavioral health conditions—they are the result of a lifetime of behaviors, lifestyle choices, physical and mental health challenges, and their environments. Caring for the whole person requires patient-centered care; however, effective care coordination is no easy feat, especially as patients are continuously moving across the care continuum.

These transitions of care are known to be high-value, critical moments in one’s care journey that impact not only their experience, but their care outcomes. In this webinar, Bamboo Health will discuss how Real-Time Care Intelligence can close gaps in care for behavioral and physical health, plus break down barriers and misaligned workflows between payers and providers.

Learning Objectives

At the conclusion of the session, participants will be able to:

  • Describe how providers are innovating to increase access to and awareness of behavioral health
  • Discuss how to effectively address physical and behavioral health care gaps, especially those within the realm of transitions of care and post-discharge follow-up
  • Identify strategies to optimize resources and foster enhanced collaboration between payers and providers

Faculty

Nishi Rawat, MD, MBA, Chief Clinical Innovation Officer, Bamboo Health

Dr. Nishi Rawat began her health care career as an emergency medicine and critical care physician at Johns Hopkins. As a health care faculty member and researcher, she led quality measure development and implementation at Johns Hopkins Armstrong Institute for Patient Safety and Quality. Clinically, she experienced the frustration of locating definitive care for patients with behavioral health conditions, which led to the development in 2015 of OpenBeds, a software platform to expedite access to care. In 2018, Open Beds was acquired by Bamboo Health, where Dr. Rawat now serves as Chief Clinical Officer. She completed her residency and fellowships at Johns Hopkins and Harvard, MD at Queen's, BSc from McGill, and MBA from Georgetown.
