HAP Congratulates PA Hospitals Receiving Grants to Improve Care Coordination for Medicaid Patients

This morning, the Pennsylvania Department of Human Services (DHS) announced it awarded nearly $8 million in onboarding grant funds to increase efficiency in health care service delivery to Medicaid patients through health information exchange.
Pennsylvania hospitals, along with outpatient facilities including physician practices and nursing homes will be able to connect with the Pennsylvania Patient & Provider Network, which enables electronic health information exchange across the state. Grants are awarded to health information organizations to enable the connection of health care facilities that participate in the Medicaid Electronic Health Records Incentive Program.
