‘Engage for Health’
Tools for Health Literacy
Supporting strategies that improve patient- and
family-centered care is a priority for HAP.
This includes helping hospitals
encourage individuals to be more actively engaged in their health and care. HAP and its Pennsylvania Hospital Engagement Network
(PA-HEN) have developed Engage for Health, a series of tools to conduct a
community education program on taking an active role in your health care.
The Engage for Health program consists of several resources
that can be used together or separately, based on your needs. It includes a
slide presentation, resources to conduct a patient/provider role play exercise,
a list of additional resources, and promotional materials.
HAP members are encouraged to use
these tools to conduct their own ‘Engage for Health’ program in their
communities, as well as partner with other organizations to hold a program.
Many libraries across Pennsylvania have received the Engage for Health toolkit
and are looking for a partner.
