Factors that affect the integration of case management among and between providers will also be covered, as well as best practice strategies for ensuring that patients do not fall through the cracks.

Webinar Overview

Integrating case management across the continuum of care requires that hospitals and post-acute providers think like an accountable care organization (ACO), even if they are not one. This kind of thinking requires that patients remain at the center of the wheel around which all case management processes revolve. Case management departments must ensure that they provide an infrastructure for managing patients across the continuum that includes processes for identifying and managing high-risk patients, regardless of setting.

Webinar Objectives

At the conclusion of this session, participants will be able to:

  1. Understand how case management is applied in different settings across the continuum
  2. Discuss the vital role of case management in coordinating care across the continuum
  3. Identify strategies for eliminating barriers affecting care transitions
