A summit in the southeast region of Pennsylvania for the purpose of learning, networking, and collaborating to improve the seamless transition of care for opioid overdose survivors from the emergency department to the treatment provider.

Program Overview

Since January 2016, the Department of Drug and Alcohol Programs has been working with stakeholders to ensure a Warm Hand-off (from emergency medical care to treatment) for opioid overdose survivors, thus improving the prospect of recovery.

This regional summit is one of six half-day sessions being held across the commonwealth, bringing together individuals from public and private sectors across diverse disciplines. The session will include a presentation about a successful Warm Hand-off program in the southeast region. Attendees will participate in sessions designed to enhance peer-to-peer learning using assessment data collected in advance of this summit. Hands-on activities will provide opportunities for collaboration on the development of localized action items to address barriers to Warm Hand-off.
