Community HealthChoices (CHC), the Commonwealth’s Medicaid Managed Long-Term Services and Supports (MLTSS) program, represents a major over-haul of how Pennsylvania pays for nursing home stays, home and community-based services, and other supports for the elderly and the physically disabled older than 21.
This new program fundamentally changes how coverage is provided to Medicaid beneficiaries and those who are dually-eligible for Medicare and Medicaid. It is critically important for hospitals and post-acute providers to understand the upcoming changes and implications for providing services. CHC is being implemented in phases based on the existing HealthChoices zones and Phase 1 is scheduled to start in the Southwest on January 1, 2018.
HAP has invited leadership from the New Jersey Hospital Association to co-facilitate this workshop. New Jersey transitioned to managed long term care in 2011 and has vast experience with both the challenges of implementation and the on-going administration. This regional workshop is designed to share valuable insight with facilities just starting this transition and to provide Pennsylvania with real life “lessons learned” from the New Jersey experience. Who Should Attend
CHC will require changes in virtually all aspects of hospital and post-acute care and administration in Pennsylvania. The program is created specifically for financial officers, operating officers, administrators, discharge planners, case management staff, directors of nursing, chief medical officers, patient advocates, and providers new to managed care, however, it also serves as a good educational platform for anyone who is seeking a greater understanding of the changes to come in Pennsylvania.

Registration Cancellation Policy
No refunds will be issued for cancellations received after November 10. There is a $25 administrative fee on all cancellations. Cancellations must be received in writing. No refunds will be issued for no-shows. Substitutions are permitted.
