The Pennsylvania Continuous Service Readiness Program (CSRP) is a joint effort between Joint Commission Resources (JCR) and The Hospital & Healthsystem Association of Pennsylvania (HAP) to structure and implement a program by which hospitals can receive education, consultation, and feedback on an ongoing basis from JCR and The Joint Commission (TJC) staff.

Case Management Across the Continuum of Care: Acute Care and Community Case Management Finally Meet!

April 30, 2019 – 10:30 a.m. to 12 p.m.

This program will teach you how to coordinate the patient’s transition from health care systems to other settings, such as moving from the hospital to rehabilitation and home settings. There will be a discussion on how to involve the patient and family in decisions regarding care. Factors that affect the integration of case management among and between providers will also be covered, as well as best practice strategies for ensuring that patients do not fall through the cracks.

HAP is offering this webinar to provide its members with an understanding of how new guidelines affect their practice and what their responsibilities are under the Child Abuse Prevention and Treatment Act and Act 54.This webinar will also provide HAP members the opportunity to speak directly with staff from the Pennsylvania Department of Human Services and the Pennsylvania Department of Health.

Collaborative Opportunities to Advance Community Health (COACH) Meeting

May 8, 2019 - 9 a.m. – noon

At this meeting, we will share updates from COACH workgroup activities, discuss CHNA efforts, and provide updates about external initiatives relevant to the COACH collaborative. We will also include a workshop session as part of a series focused on return on community investment, community benefit planning, and the development of a shared regional vision and approach to address community health needs.

Overview of Medicare’s VBP Program and Analysis Webinar

May 28, 2019 - 12 p.m. - 1 p.m.

HAP is hosting a two-part educational webinar program to better understand Medicare’s quality-based payment reforms. The webinars will be conducted by HAP’s data analytics partner, DataGen. Datagen is a subsidiary of the Healthcare Association of New York State (HANYS) and contracts with 47 state hospital associations and eight multi-state health systems to research and analyze the financial impact of regulatory and legislative changes on Medicare reimbursement to hospitals and health systems.
